The term safety culture was coined in 1988 in a report on the Chernobyl disaster that impacted more than 500 thousand of workers the report noted that the lack of KNOWLEDGE and UNDERSTANDING of risk and safety by the employees and the organization contributed to the disaster. It was noted that organizations with a positive safety cultures are characterized by open communication founded on mutual trusts shared perceptions of the importance of safety, confidence in the effectiveness of measures plus strong leadership and personal accountability
